Join us for an engaging capacity-building webinar that introduces social prescribing—a holistic, person-centred approach that connects individuals to non-medical supports and community-based resources to improve their overall health and well-being.

This session will explore the growing body of evidence on the effectiveness of social prescribing, its benefits in addressing social determinants of health, and the important role it plays in integrated care models across sectors.

Learn about the Links2Wellbeing initiative, a collaborative program from the Older Adult Centres’ Association of Ontario (OACAO), and the Alliance for Healthier Communities supporting older adults across the province through social prescriptions. We'll discuss how the program works, current practices, key successes, common challenges, and practical implementation strategies that can help your organization become part of this impactful movement.
