Social Inclusion Through Social Prescribing Introductory Webinar

The overall Social Inclusion Through Social Prescribing project goal is to increase social engagement and the inclusion of older adults by connecting them to their local seniors’ centres on the recommendation of their primary health care provider. Volunteer ambassadors, knowledge experts of their centres, will link older adults with programs offered at the centre.

Whether you just want to learn more about social prescribing and the project or whether you already know that you want to be a participating site, we invite you to join us on October 7^th to begin the journey into the world of social prescribing.
